Free Play Football Consent Form

We are pleased to be able to offer FREE PLAY FOOTBALL CLUBS at multiple venues in South Oxfordshire!

Please could we ask you to fill out the following form to consent to your children taking part in the Club. This form does not commit you to anything but provides us with information for safeguarding purposes. Once we have received your form one of our team will be in touch to confirm your child’s place(s). If you have multiple children please fill out one form per child. We are unable to accept children on site without a consent form.

Our Free Play Football Clubs are held on:

Mondays at Carterton Recreation Ground, OX18 3DD from 4:30-5:30pm

Thursdays at Grove Rugby Club, OX12 0FL from 4:30-5:30pm

Fridays at East Hanney Recreation Ground, OX12 0JL from 4:30-5:30pm

Please note these are not coached sessions but pick up games for children to play freely.
